Mutual Agreement for CME Recertification Reimbursement

Agreement dated this day ________________between ___________________________ hereafter referred to as the "Student" and Bassett Healthcare' s Center for Rural Emergency Medical Services Education, hereafter referred to as the "Center" in conjunction with The Mary Imogene Bassett Hospital, hereafter referred to as the "Hospital."

WHEREAS, the Student wishes to retain the Center for the purposes of EMS Recertification through the NYS CME Recertification Program (The Pilot Program). The student assures enrollment of the CME recertification program through an approved EMS agency.

WHEREAS, the Hospital and the Center, in turn, agree as follows:

  1. The Hospital shall provide qualified Instructors, for students of the Center in accordance with the memorandum authorizing the Department of the Emergency and Trauma Services management to use the facilities and staff of the Hospital.
  2. The Center shall schedule, and provide such schedule to student, core content classes to meet the requirements of the CME core content as defined in the CME Recertification Program Manual provided by NYS.
  3. Reimbursement: Students must place initials indicating which option they choose:
    1. The Student may elect to pay the tuition, one hundred fifty dollars ($150.00) at the beginning of each class. Payments can be made by check or money order payable to Bassett Healthcare or;
    2. The Student may permit the Center to voucher the State of New York for any applicable fees related to expenses incurred by the Center on behalf of the Student. If the State of New York's Department of Health, Bureau of Emergency Medical Services reimburses the Center for expenses, the student is relieved of tuition. In the event the student does not complete the core content requirements with the Center within one year from the start the student is liable for the tuition. In the event that the students EMS agency vouchers the state for reimbursement the student is liable for the tuition. In the event the State of New York denies reimbursement the student shall be liable for the tuition.
  4. Modifications: This is the entire agreement. There shall be no oral modifications to this agreement, and any modifications or amendments of the terms of this agreement shall be binding unless executed in writing by the parties hereto. The terms of this Agreement supersede any oral representations previously made.
